About the role
We are looking for a Transition Support Worker who will be based at our school in Nottingham City Centre, Sutherland House School. This is an exciting project to support autistic young people and adults across the East Midlands.
We currently have a vacant post focusing on supporting young people and adults transition into adulthood to secure meaningful long-term outcomes.
The purpose of the role is to provide a consistent presence in the life of young people aged 14-25 years of age to enable a supportive transition into adulthood.
The role will enable long-term positive outcomes, provide signposting for support available including housing and benefits and assisting in employment or further education opportunities for autistic young people and adults including supporting access to work.
What will you do
Our Transition Support Workers will provide one-to-one support to autistic individuals. This will include both face-to-face and online meetings and the securing of long-term outcomes for those who access the Transition Support Package.
Purpose of the Post:
To be a consistent presence in the life of a young person into adulthood (aged 14-25 years) to support a meaningful and positive transition into adulthood.
To work to support young people to identify ambitions for the future.
To support young people from the age of 14 years to enable fulfilment of ambitions into adulthood.
To work with internal and external partners to identify and work with to achieve outcomes e.g. accommodation, benefits, employment, education, training, apprenticeships.
To advocate on behalf of young people internally and externally.
To liaise with the external evaluation team to in design and implementation of the project to provide a meaningful evaluation report at the end of the two-year project.
