In this rewarding role, you will support students as they transition into higher education, employment, or independent living - helping to plan their journeys, build confidence, and achieve positive outcomes.
Key Responsibilities:
Plan and coordinate individual transition pathways for graduating students
Organise and lead transition meetings with students, families, and external partners
Develop and maintain strong relationships with employers, education providers, and supported living services
Deliver engaging sessions on CV writing, applications, and next steps
Provide guidance to students progressing into Higher Education, including support with UCAS
Track student destinations and maintain alumni engagement
Create resources such as transition guides and student success stories
Provide general administrative and operational support across the College
About You:
We are looking for a proactive and organised individual with excellent communication and interpersonal skills. Confident working with a wide range of stakeholders and you will manage competing priorities effectively. You are comfortable working independently as well as part of a team, with strong administrative, report-writing, and IT skills. Above all, you must have a genuine interest in supporting young people, particularly those with additional needs or disabilities.
